دايما بنسمع علي الرجل الاخضر

   وبنشوفه في التلفزيون بس عمرنا سمعنا عن الراجل الاصفر

ده بقي عندنا في الطب والراجل ده بقي تعالوا نشوف هو بقي كده ازاي

⟱Jaundice :

Yellowish discoloration of skin,sclera

&mucous memebrane du to increase serum bilirubin>3mg%

طيب يبقي كده عرفنا ان الموضوع كله بيعتمد علي الـ  bilirubin   

تعالوا نشوف  قصة حياته بقي

 اولا في عندنا الـ RBCs  ما بتتكسر في  spleen haem

بتتحول الي (Fe&Bilirubin)

  bilirubinال بياخد صفه insoluble (unconjugated)   وبيتحد مع الـ albumin 

وبيسافروا مع بعض في  blood stream

لحد لما يوصل للـliver  هيلاقي  z&y بروتين بيغروا بيدخل الـliver  وبيتحول فيها ويتشقلب كيانه ويتحول

لـ (water soluble (conjugated bilirubin)

بعد ما اتظبط في الـliver  واخد متعته يطرده الـliver  في الـbile  ويدخله الـgut  

ويتحول فيها stercobilinogen  

  stercobilinogen بيتخذ ثلاثة طرق للنهاية

 اescape to kidney  ويخرج علي هيئة  urobilinogen

وايضا excreted in faeces (sterco bilin)

و reabsorption from gut 》》enterohepatic  circulation

تمام كده عرفنا رحلة الــ bilirubin   والي اي مشكله فيها هتعملي jundice

ناتي لمشاكل الاكل ):

1. hemolytic  jaundice

A_Pathogenesis:

Increase hemolysis of RBCs》》  بيزود الــ Hemobilirubin 

طيب ده هيسبب مشاكل مع الــliver  الغلبان وموش هيقدر ياخد كل الكميات دي

So part of it retained in the blood 》》incr serum bilirubin》》jaundice

طيب والكميات الي دخلت للـliver  

هتتحول لـconjugated bilirubin》》stercobilinogen  

اللي هيعمل dark stool

وهيعلي معاك بردو urobilinogen بس موش هياثر علي urine  عشان هو colorless

B_Clinical picture:

1.Jundice mild (lemon yellow)

2.Urine normal

3.Stool dark

4.Feature of hemolytic anemia

C_Investigation:

1.In serum :unconjugated bilirubin⇈

2.In stool :stercobilinogen⇈

3.In urine :urobilinogen⇈   

ندخل علي تاني نوع (مشاكل الطرد ):

2. Obstructive  jaundice

A_Pathogenesis:

Decreas of excretion of conjugated bilirubin into intestine due to obstrucrion

ده علي اساسه هيعمل حاجتين مهمين

*stercobilinogen بتقل pale stool

*conjugated bilirubin هترجع للدم تاني عشان السكه مقفوله

وبالتالي هتعلي serum bilirubin 》》jaundice

*Conjugated عنده قدره يدوب في الماء وبالتالي هيعمل dark urine

في حاجات تانيه بتحصل مصاحبه obstruction

ناقص bile excretion in intestine  

هيعمل steatorrhea

 Bile salt هيرجع تاني للدم وبيظهر معاك in urine

تمام نيجي بقي لشكل العيان

B_Clinical picture:

1.Jaundice :deep(olive green)

2.Urine :dark =bilirubin

3.Stool :very pale

4.Other feature of obstructive jaundice

Staetorrhea

Prolonged chlestasis》》cirrhosis

وفي حاجات تانيه كتير

نيجي بقي للــ

C_Investigation:

1.In serum :(conjugated /cholesterol/alk.phosphate) increased

2.In stool (stercobilinogen ⇊ )

3.In urine (urobilinogen⇊

4.Bilirubin is present

(نيجي بقي لاخر نوع معانا مشاكل الطرد والاكل )

 3. Hepatocellular  jaundice

A_Pathogenesis:

1.Decreased of uptake ,conjugation&excretion of bilirubin

طيب كل واحد بيعمل مشكله

2.Decrease uptake 》》 increased of 

un conjugated bilirubin

3.Decrease excretion 》》 increased of conjugated bilirubin 》》dark urine

&decreased of stercobilinogen 》》pale stool

B_Clinical picture:

1.Juandice: orang yellow

2.Urine:dark

3.Stool:pale

4.Features of liver failure

C_Investigation:

 هتلاقي فيه

1.in serum increased both conjugated &unconjugated bilirubin وده مميز ليه

2.In stool :  decreased sercobilinogen

3.in urine increased urobilinogen &bilirubin is present
